Payment, Cancellation & Refund Policy

Balance Hormone & Wellness
Effective date: November 1, 2026

This policy explains our consultation fees, optional Balance Journey memberships, payment timing, cancellations and refunds. Please review it before booking or enrolling. We will confirm your selected services, fees and membership dates before charging you.

Consultation and booking deposit

Your initial 60-minute virtual consultation costs $300. A $100 deposit reserves your appointment and is applied toward that fee. The remaining $200 is due at your visit. The consultation fee is separate from membership fees and is not credited against monthly or prepaid ongoing-care charges. Membership is optional; a consultation does not automatically enroll you.

After booking, we will send instructions for setting up your patient portal and completing intake forms. If labs are recommended before your consultation, we will explain the testing and payment options.

Balance Journey payment options

Care Only costs $150 per month, or $810 prepaid for six months of ongoing care. Prepayment saves $90 compared with six monthly payments totaling $900.

Testosterone Included costs $200 per month, or $1,080 prepaid for six months of ongoing care. Prepayment saves $120 compared with six monthly payments totaling $1,200.

The 10% prepayment discount applies only to ongoing membership care, not to the consultation, labs or separately billed medications. Including the $300 consultation, six months of Care Only costs $1,200 with monthly billing or $1,110 with prepayment. Testosterone Included costs $1,500 with monthly billing or $1,380 with prepayment. Labs and other separately billed services or medications are additional.

When membership payments begin

Your first membership payment is due when your membership begins, whether you enroll at your consultation or later. Monthly payments are billed in advance on the agreed monthly billing date. Six-month prepayments are due in full at the start of the prepaid term. We will confirm your start date, billing date or prepaid end date, and payment authorization before enrollment.

Monthly membership renews automatically only with your affirmative authorization. We provide a confirmation you can retain, including the amount, billing schedule and cancellation instructions. We provide renewal notices required by applicable law. A six-month prepayment is a single payment and does not automatically renew or convert to monthly billing.

What ongoing care includes

Membership includes personalized perimenopause and menopause care, scheduled follow-up consultations, prescription management and treatment adjustments, and patient-portal support directly with Vanessa. Visits and refills follow your individual treatment plan. Monthly billing does not mean a visit or medication shipment occurs every month. Membership is not health insurance and does not cover emergency care.

Medications and laboratory testing

Care Only includes testosterone management when clinically appropriate, but medications are purchased separately through a pharmacy. Testosterone Included covers the eligible prescribed initial testosterone supply from a compounded pharmacy, subsequent covered refills from a compounded pharmacy and standard shipping during active membership. Your included formulation, quantity and refill schedule are confirmed before enrollment. Other medications and laboratory tests are billed separately.

Prescriptions are based on an individualized clinical evaluation. Enrollment does not guarantee testosterone or any other prescription. A licensed pharmacy dispenses prescribed medication. Compounded medications are not FDA-approved. We discuss relevant risks, benefits and alternatives before prescribing and obtain appropriate treatment consent. If testosterone is not appropriate or is discontinued, we will discuss the appropriate care option and confirm any billing change with you.

The cash price for the core lab panel is $125. Additional or different tests may cost more; we confirm the applicable cash price before testing. If your laboratory bills insurance, coverage and out-of-pocket costs depend on your plan. Balance’s consultation and membership services are cash-pay professional services; a laboratory’s use of insurance does not mean those services are covered.

Appointment cancellations and missed visits

Please give at least 24 hours’ notice to cancel or reschedule an appointment. For an initial consultation canceled with at least 24 hours’ notice, your $100 deposit may be refunded or transferred to a new appointment. If you cancel with less notice or miss the initial appointment, the $100 deposit is retained as the late-cancellation or missed-appointment fee. The remaining $200 consultation balance is not charged for a visit that did not occur.

For membership follow-up visits, please provide the same 24-hour notice. A missed visit does not pause membership billing. No separate missed follow-up fee is charged under this policy. Contact us promptly about an emergency or technical problem; we will review the circumstances. If Balance cancels your consultation and you choose not to reschedule, payments for that unprovided consultation are refunded.

Canceling monthly membership

You may request cancellation through the patient portal or by email to hello@balancehormonewellness.com. No telephone call or in-person visit is required. Include your name and that you wish to cancel; do not send medical details by ordinary email. We will confirm receipt, the effective cancellation date and any final payment or refund. The date we receive your request controls, even if our confirmation arrives later.

Cancellation takes effect 30 calendar days after we receive your request, unless an earlier date is agreed in writing or required by law. Membership services remain available through the effective date. You are responsible for charges through that date, not for a new six-month commitment. Any final partial monthly period is prorated using the number of calendar days in that billing period. Payments covering days after the effective cancellation date are refunded. Fees for elapsed membership periods are not refunded solely because you did not schedule a visit or use every service.

Canceling prepaid care and calculating refunds

You may cancel prepaid care using the same contact methods and 30-day notice described above. We refund the unused portion after your effective cancellation date. Your refund equals the prepaid amount multiplied by the unused calendar days remaining in your term, divided by the total calendar days in that six-month term. We do not recalculate elapsed care at the higher monthly rate or deduct a separate medication charge from this refund.

For example, if half of your prepaid term remains after cancellation takes effect, the refund is $405 for Care Only or $540 for Testosterone Included. Actual refunds depend on the dates of your term and effective cancellation. The completed consultation is not refundable. We will provide an initiate a refund to the original payment method within 10 business days; your financial institution may take additional time to post it. Refunds required by law are not limited by this policy.

Continuing care after six months

Before your prepaid term ends, we will discuss your ongoing care needs and available payment options. Continuing care requires your agreement to the applicable fees and terms. We do not automatically charge for a new prepaid term or begin monthly billing. A maintenance option, if offered and appropriate, will be explained before you decide. There is no guaranteed maintenance price under this policy.

Clinical transitions and changes to service

Canceling membership does not replace a clinical plan for safely continuing, changing or stopping treatment. Contact us about follow-up needs and transfer of care. Refills depend on clinical appropriateness and required monitoring. If Balance must end care or cannot provide the agreed services, we will communicate the effective date and appropriate transition arrangements and refund unused prepaid services from the service-end date; the ordinary 30-day cancellation notice does not extend charges beyond that date.

If a payment fails, we will notify you so you can update your payment information. We will discuss any effect on ongoing services and necessary transition arrangements rather than treat a failed payment as an instruction to stop medication. Any material price or policy changes will be communicated in advance, with cancellation information and any authorization required by law. Agreed prepaid rates remain unchanged for the term already purchased.

Good faith estimates and billing questions

If you do not have insurance or are not using insurance for your care, you have the right to a Good Faith Estimate of expected charges. We provide estimates as required when you schedule care and upon request. This pricing page does not replace an individualized written estimate. Ask separately billing laboratories and pharmacies for estimates of their charges.

If a bill from a provider is at least $400 more than that provider’s Good Faith Estimate, you may qualify for the federal patient-provider dispute resolution process. Generally, you must begin that process within 120 calendar days of the initial bill. Visit www.cms.gov/nosurprises for information about eligibility and your rights. Nothing in this policy waives applicable consumer or patient protections.

For payment questions, cancellations, refunds, or an accessible copy of this policy, contact Balance Hormone & Wellness through your patient portal or email hello@balancehormonewellness.com. Do not include sensitive medical information in ordinary email.
